According to a DHS spokesperson, the hospital aides were terminated after a female patient reported she had been raped by a male patient at the facility on April 30, 2012.
The employees, according to DHS, failed to follow patient monitoring policies.
Three lower level staff were also disciplined to a lesser degree.
The case files have been turned over to the Pulaski County Prosecuting Attorney.
DHS is currently working to compile a "root cause analysis" report, defining where the breakdown occurred and what policy changes need to be implemented.
